Cara Usul NISN Baru

Ada tiga layanan Dapodik (Data Pokok Pendidikan) mengenai NISN (Nomor Induk Siswa Nasional), yaitu:

• Pengajuan NISN Baru
• Pengajuan Pindah Keluar
• Perbaikan Biodata Siswa

I. Perbaikan Biodata/Tingkat Siswa
Mengisi formulir B.3.1. Formulir memuat keterangan:

• Data yang mengajukan, berisi data (1). Nama  (2) Status (pilih: orang tua, wali siswa, siswa)

• Data Mutasi Siswa, keterangan yang harus diisikan adalah : NPSN (cari NPSN klik di sini, Nama Sekolah, NISN, Tingkat (Kelas), Nama Siswa, Jenis Kelamin, Tampat Lahir, Tanggal Lahir, Alamat.

Formulir ini ditandatangani oleh yang mengajukan. Dan, di bagian bawah formulir terdapat keterangan sebagai berikut:
1. Harap melengkapi dan menandatangani formulir ini,
2. Formulir diserahkan kepada Operator Data Pokok Pendidikan (Dapodik) Sekolah atau Dinas Pendidikan Kabupaten/Kota setempat.
3. Pastikan Anda menerima CETAK TANDA BUKTI Perbaikan Biodata / Tingkat Siswa dari Operator Dapodik bersangkutan.
4. Jika mengalami kendala, Harap menghubungi Pusat Pelayanan Dapodik

II. Pengajuan Pindah Keluar/Pindah Sekolah
Mengisi formulir B.3.2. Formulir memuat keterangan:

• Data yang mengajukan, berisi  keterangan (1). Nama  (2) Status (pilih: orang tua, wali siswa, siswa)

• Data Mutasi Siswa, keterangan yang harus diisikan adalah : NISN, Nama Siswa, Sekolah asal Asal, Tampat Lahir, Tanggal Lahir, NPSN (cari NPSN klik di sini) asal, Nama Sekolah Tujuan, No. Surat Mutasi, NPSN Tujuan, Tingkat (Kelas),

Formulir ini ditandatangani oleh yang mengajukan. Dan, di bagian bawah formulir terdapat keterangan sebagai berikut:
1. Formulir ini hanya berlaku di wilayah Kab/Kota asal siswa
2. Harap mengisi dan menandatangani formulir ini, dan menyertakan surat keterangan mutasi dari sekolah asal.
3. Formulir diserahkan kepada Operator Data Pokok Pendidikan (Dapodik) Dinas Pendidikan Kabupaten/Kota setempat.
4. Pastikan Anda menerima CETAK TANDA BUKTI Pengajuan Pindah Keluar Siswa.

III. Pengajuan NISN Baru (Hanya berlaku bagi siswa selain kelas 1, 7 dan 10)

• Data yang mengajukan, (1). Nama  (2) Status (pilih: orang tua, wali siswa, siswa)

• Data Mutasi Siswa, keterangan yang harus diisikan adalah : NPSN(cari NPSN klik di sini) , Nama Sekolah, Tampat Lahir, Tanggal Lahir, Alamat, Nama Siswa, Tingkat (Kelas), Jenis Kelamin

Formulir ini ditandatangani oleh yang mengajukan. Dan, di bagian bawah formulir terdapat keterangan sebagai berikut:
1. Hanya berlaku bagi siswa selain kelas 1, 7 dan 10.
2. Harap mengisi dan menandatangani formulir ini,
3. Formulir diserahkan kepada Operator Data Pokok Pendidikan (Dapodik) Sekolah atau Dinas Pendidikan KabupatenKota setempat.
